To change your name and/or address please email the Benefits Office @ firstname.lastname@example.org. You need to include your name, SS#, school location, and what type of change you have.
A packet will be forwarded to you with everything you need to have your change(s) made. Once you have returned your packet, the change will be forwarded to all district office departments and insurance carriers. You are responsible for making this change at your school level.
